Food Truck Entry Form

Welcome to The Great Umpqua July 4th Food Truck Competition entry page.

Registration Fee: All food truck registrations are required to pay a $250 non-refundable entry fee to secure their spot upon notification of acceptance.

Submit Photo: Please attach a recent photo of your Food Truck at an event or your current serving location to: bsmayber@co.douglas.or.us

Permit: Each vendor is required to obtain the necessary food permit through the Douglas County Health Department. A copy of the food permit will be required to be posted on-site during the event.

Electrical, Sewer & Water Hook-up: There are spaces available with Power/Sewer/Water, Power/Water, Power only 20amp or 50amp. Space is also available for self-contained Food Trucks.

Load-In: Event load-in Monday 8am to 10pm on July 3rd and on Tuesday 8am and must be completed by 11am.

Load-Out: Event load-out is Tuesday, July 4th at 8pm to 10pm or Wednesday, July 5th at 8am - Noon.
